It incorporates client demographic data, treatments, services, reporting and a resource directory.

Eligibility Module
CareScope Eligibility prescreens clients for programs and services that improve health and quality of life through a speedy eligibility process. A program administrator can utilize CareScope Eligibility to define the qualifying criteria for any federal state or local program, including those that cover human, social, or medical services.

Evidence shows that the qualification process can be long, time-consuming, tedious and plagued by complexity, errors and guesswork. With CareScope Eligibility your precious time and resources are focused on the core needs of the
client because the eligibility process is fully automated and virtually paperless. The automated tasks include determinations of income, household size and FPL percentage. Forms can be pre-populated and referrals generated with as few as 2 mouse clicks. Clients are assessed for multiple-program eligibility in a single pass. The guesswork is eliminated – a search returns all programs for which the client is eligible and the client can be quickly linked with the appropriate resources. Even potential clients can be immediately served, since with minimal data entry they can be pre-qualified through the “Quick Eligibility” feature.
Case Management Module
CareScope features case management tools to automate assessments, track goals and develop care plans. Follow-ups and caseloads are managed quickly and easily through the user’s Quick View.

CareScope helps identify the obstacles that impede the client’s access to better health so the case manager can address them through education and referrals to relevant programs. Pertinent case management tasks can be performed from within the care plan. Care planning is automated. The assessment summary drives the automatic triggering of pre-determined goals and interventions. CareScope can automate the interventions that are tied to the goal and then track results.
HMIS Module
CareScope HMIS is an effective weapon in the mission to eradicate homelessness. The CareScope Homeless Management Information System (CareScope HMIS) combines with the other modules of CareScope software to provide a unique and holistic framework for serving the homeless. It advances the transition from homelessness to permanent housing through the identification and utilization of community resources.

From a health care perspective, the homeless are a high-risk population to manage. CareScope HMIS tracks homeless data as well as medical data to facilitate continuity of care. Access and health concerns are addressed in coordination
with homeless and environmental issues. When they have a safe place to go and access to care, the homeless are less likely to end up in the ER or admitted to the hospital and more likely to take care of their health issues.

As with all CareScope modules, HMIS is eminently customizable. Required fields can be identified so that only pertinent data must be keyed. There are over 100 canned reports, including the APR, AHAR, and other widely-used homeless reports. An ad hoc feature expands reporting capability with the option to build reports using selected fields.
Community Module
CareScope Community is a web portal 2-1-1 system that connects the entire community with programs, services and resources. With its resource directory, CareScope Community is an adjunct to 2-1-1 calling systems, providing users with information about and referrals to resources such as food banks, clothing assistance, shelters, rent assistance, utility assistance, health insurance programs, Medicaid and Medicare and health education.

All too frequently those in need don’t utilize the programs and services that are available because they just don’t know about them. CareScope Community paves the way to access by providing education about available services.
Resources are displayed by facility and clients are quickly matched with services.

With a minimum of data entry, potential clients can be pre-qualified through the “Quick Eligibility” feature. This rapid assessment process quickly eliminates non-qualifiers and provides feedback about why they weren’t eligible.

CareScope Community’s on-line referral center manages the referral process. It tracks referral status and ensures that the client doesn’t get lost in the shuffle. With a minimum of data entry, community partners can participate in the referral process.
MIS-to-Go Module
With CareScope MIS-To-Go the power of CareScope becomes portable. A full-featured, stand-alone version of CareScope, CareScope MIS-To-Go offers remote access to a database populated with client information on laptops and other portable computers. CareScope MIS-To-Go creates a mobile workforce. It arms case managers with a tool to extend their effectiveness and increase their understanding of client needs.

Field workers have access to data from anywhere in the community and can update that information during the client visit. Later, the most current client data can be uploaded to the main server and any changes made by other users can
be downloaded to the user’s system. Case workers can visit homebound patients or take the laptop with them to an enrollment event.

CareScope MIS-To-Go doubles as a disaster preparedness tool. A copy of the database can be stored locally for complete redundancy. In the field or in the office, the user can work off-line from anywhere without a functioning network or even an internet connection. During an emergency, a real time snapshot of service and medical history, allergies and other critical client data can be accessed off-line.
Volunteer Physician Tracking Module
The first step in building the health pathway for the underserved is to provide a medical home. Subsequently, the client can receive necessary medications, see a specialist and undergo diagnostic testing. Visits to the emergency room for primary care are replaced by routine appointments with a volunteer clinic or physician. By connecting the client with the services of a primary care physician, CareScope Volunteer Physician Tracking facilitates access to all the other levels of health care.
